If the head fails to deliver despite additional manoeuvres, then consideration should be made for performing cervical incisions. These are known as Duhrssen's incisions.b. Cervical incisions should be made a 2 'o clock and 10 'o clock to avoid lateral extension of the incision involving the descending cervical vessels. An additional incision at 6 'o clock position is rarely needed.c. The main difficulties when performing cervical incisions for head entrapment at breech delivery are achieving adequate analgesia and exposure. There is a significant risk of haemorrhage, the cervical incision may extend upwards within the broad ligament causing broad ligament haematoma.
