Ergometrine should not be given to women with high blood pressure,Controlled Cord Traction# Clamp the cord close to the perineum. Hold the clamp with right hand and place your left hand above the pubic bones to stabilize the uterus and to apply counter pressure during traction on the cord. Slight tension is kept on the cord and a strong uterine contraction is awaited. When a contraction comes the parturient is encouraged to push and a gentle traction is applied to the cord continuing to apply counter-pressure on the uterus at the same time.Uterine Massage# Immediately after expulsion of the placenta uterus is massaged until it is contracted. Palpation of the contracted uterus is done even 15 minutes for two hours to see if the uterus becomes relaxed and uterine massage is repeated when the uterus becomes relaxed.Note: For the active management of the third stage of labour oxytocin is preferable over ergotamine as oxytocin is more stable and has lesser side effects.
