Correct Answer: IgA nephropathy
Description: Ans. (a) IgA nephropathyRef: Harrison's 17h ed. Ch. 277* IgA nephroathy also known as Bergers disease. IgA antibodies get deposited in mesangial cells and cause dysfunction of mesangial cells and lead to microscopic/ gross hematuria.* It is classically characterized by episodic nature and is one of the most common forms of glomerulonephritis worldwide.* The two most common presentations of IgA nephropathy are recurrent episodes of macroscopic hematuria during or immediately following an upper respiratory infection in children (Henoch-Schonlein purpura) or asymptomatic microscopic hematuriamost often seen in adults.NOTE: GROSS HEMATURIA can be seen with IgA nephropathy and sickle cell disease.MCC of microscopic/gros hematuria: IgA Nephropathy (Berger's disease).* Minimal change disease or lipoid nephrosis is the most common nephrotic syndrome in children. Presence of abnormal T-cell release cytokines and cause damage to visceral cell damage i.e., effacement of podocyte which leads to massive proteinuria.* In PSGN the classic presentation is an acute nephritic picture with hematuria, pyuria, red blood cell casts, edema, hypertension, and oliguric renal failure, which may be severe enough to appear as RPGN.
