Characteristic of a child with acute post-streptococcal glomerulonephritis is: March 2005

Correct Answer: Raised ASO titre
Description: Ans. C: Raised ASO titre A mild anemia (normocytic, normochromic) is common in persons in the early phase of acute glomerulonephritis; its degree tends to parallel the degree of extracellular (ECF) volume expansion. Erythropoiesis may decline in the aftermath of acute glomerulonephritis, paicularly in individuals with severe cases. WBC and platelet counts are usually normal, although an occasional patient exhibits a leukocytosis; rarely, a mild thrombocytopenia may be present. A few patients have hypoproteinemia and hyperlipidemia. A nephrotic picture may be seen in patients with poststreptococcal acute glomerulonephritis. Numerous laboratory tests can be used to measure antibodies to various streptococcal antigens (e.g., ASO, AH, anti-DNase B) or to combinations of antigens (e.g., streptozyme test). The ASO titre is increased in more than 80% of cases. Anti-DNAse B is raised in cases of streptococcal skin infection. The level of serum C3 is low in 90% of patients but normalizes by 5-6 weeks.
