Description: *Vitamin B12 neuropathy is large fiber sensory type (dorsal column involvement) There are many possible causes of small fiber neuropathy. The most common causes are diabetes mellitus or impaired glucose intolerance.Other possible causes include hypothyroidism, Sjogren's syndrome, systemic lupus erythematosus, vasculitis, sarcoidosis, Vitamin B12 deficiency, celiac disease, Lyme disease, HIV, Fabry disease, Charcot-Marie-Tooth Disease, amyloidosis, Ehlers-Danlos Syndrome, and alcohol use disorder. A 2008 study repoed that in approximately 40% of patients no cause could be determined after initial evaluation. When no cause can be identified, the neuropathy is called idiopathic. A recent study revealed dysfunction of a paicular sodium channel in a significant poion of the patient population with an idiopathic small fiber neuropathy. Ref Harrison20th edition pg 2789
