Description: D i.e. Intraocular pressure measurement Painful loss of vision, circumcorneal (deep ciliary) congestion points towards the diagnosis of posttraumatic secondary glaucoma. So the first investigation would be measurement of IOP.Q Angle recession glaucoma may show early rise (with in hours) of IOP due to protaglandin activity or delayed rise due to trabecular damage. Post traumatic secondary glaucoma may complicate perforating as well as blunt injuries. It develop by one or more of following mechanism. i) Inflammatory glaucoma due to iridocyclitis; ii) Due to intraocular haemorrhage; iii) Lens induced glaucoma due to ruptured, swollen or dislocated lens; iv) Adherent leucoma & peripheral anterior synechiae formation; v) Epithelial or fibrous in growth; vi) Angle recession (cleavage) glaucoma due to disruption of trabecular meshwork f/b fibrosis.
