The measurement of aerial diastolic pressure with a sphygmomanometer may be complicated by the fact that systolic sounds are frequently heard with the cuff completely deflated. As the disease progresses and the LV end-diastolic pressure rises, the aerial diastolic pressure may actually rise as well, because the aoic diastolic pressure cannot fall below the LV end-diastolic pressure. For the same reason, acute severe AR may also be accompanied by only a slight widening of the pulse pressure.
