Description: The incidence of proximal gastric cancer is increasing. Carcinoma proximal stomach is not associated with H. pylori infection unlike cancers of body and distal stomach. Management of gastric carcinoma * Early growth in pylorus - Lower radical gastrectomy with the removal of tumour, proximal 5 cm clearance, nodal clearance, greater and lesser omentum, distal pancreas and spleen ( now not regularly removed; it is removed to clear splenic nodes - one of the node stations) and Billroth II anastomosis or Roux-en-Y anastomosis is done. Postoperatively adjuvant chemotherapy should be given - 5 fluorouracil, mitomycin, epirubicin, cisplatin, oxaliplatin, capecitabine. Ref: SRB&;s manual of surgery,3 rd ed, pg no 788
