Acute humoral rejection o Characterized by nectrotizing vasculitis that involves Significant endothelial damage Neutrophilic infiltration Deposition of immunoglobulins, fibrin and complement Thrombosis o Intimal thickning due to proliferation of fibroblasts myocytes and foam cells. o Necrosis of renal paranchyma. o Presence of C4b in peritubular capillaries is the defenitive indication of humoral rejection. 3)Chronic rejection o It results from prolong slow damage to endothelial cells of the vessels, resulting in progressive obliteration of lumen. o It is characterized - Interstitial infiltrate of mononuclear cells containing large number of plasma cells and eosinophils that cause interstitital fibrosis. Vascular intimal fibrosis, principally in coical aeries. Tubular atrophy (contrast with acute humoral and cellular rejection, where coical necrosis predominate). Duplication of basement membranes of glomeruli --> chronic transplant glomerulopathy.
